- WFD Application for Services
- Affidavit of Residency
- Social Security Card (For Applicant ONLY)
- Birth Certificate (For Applicant ONLY)
- If 17 YOA or Under – Parental Consent (pg. (Apply early if you need assistance with obtaining Documents)
- Signed Plan (This will be done with your case worker)
Late applications will not be accepted, and only complete applications will be considered contingent upon available funds.
